Medical Referrals

At Affinity Home Health Care, we know your time is important.  Our goal is to make the referral process as easy as possible.  Please feel free to call us, 713-947-2277 or  we will do a chart review, meet with the patient and family, explain our services and coordinate and order any DME or private services.

If you prefer you can utilize our Fax Referral form.  Once the referral is received and orders are obtained, we will contact the patient and see them within 24-48 hours.
