Diabetes affects millions of people and it is the 7th leading cause of death in the U.S. Complications of diabetes include heart disease and stroke, hypertension, blindness, kidney disease, amputations and neuropathy. Affinity Home Health Care's Diabetes Program includes educating the patient and family about medication, how to monitor blood sugar levels, dietary changes and when to seek medical attention in coordination with the patient’s physician. It is specifically tailored to provide care in the patient’s home and is based on the standards of care established by the American Diabetes Association.
